Molten Volleyball Club 2008/2009 Junior Boys Tryouts to be held on September 1, 2008

Please complete all the Forms listed below and bring with you a $10 Tryout Fee per player

Practice Locations

Junior Boys' practices will be held on Tuesdays and Thursdays from September 2008 through November 13, 2008.
After November 13, 2008, practices will shift to Mondays and Wednesdays.  Practice locations will include the following
facilities: Chandler-Gilbert Community College Williams Campus, Mesquite Junior High, Highland Junior High, 
Gilbert Junior High, South Valley Junior High, and Desert Ridge Junior High. Each team will be assigned a primary
practice location at the beginning of the season.  
 
Practices for all Junior Boys' teams will start on September 9, 2008.

Age Category 14 and Under, Tryouts on Monday, September 1, 2008
Tryout location will be at Chandler-Gilbert Community College Williams Campus Gym

9:30AM - 1:00PM
9:30AM Registration - Sign-in with a $10 Try-Out Fee
10:00AM Parent Meeting
10:00AM - 1:00PM Tryouts
A parent must be present during the parent meeting and during the end of the try-out session.  
Registration and a non-refundable commitment fee of $300 will be collected after selection
of teams towards the end of the tryout process. 
 
Age Category 16 and Under, Tryouts on Monday, September 1, 2008
Tryout location will be at Chandler-Gilbert Community College Williams Campus Gym

1. Molten Volleyball Club Player Info and Tryout Waiver
2. AZ Region Membership Form (2009)
3. AZ Region Medical Release Form (2009)
4. Molten Volleyball Club Photo and Internet Release Form
5. Player Participation Agreement/Commitment Form
6. Player Chandler-Gilbert Release Form
7. Copy of birth certificate (unless you are a returning Molten Volleyball Club player copy is already on file)
8. Bring at least one parent for parent meeting
9. Junior Boys 2008-2009 Fees for your files
