Molten Volleyball Club 2008/2009 Junior Girls Tryouts to be held in November 2008

Please complete all the Forms listed below and bring with you a $15 Tryout Fee per player

Practice Locations

Junior Girls' practices will be held on Tuesdays and Thursdays.  Practice locations will include the following

facilities: Chandler-Gilbert Community College Williams Campus, Highland High School, Highland Junior High, Mesquite Junior High,  

Gilbert Junior High, South Valley Junior High, and Desert Ridge Junior High. Each team will be assigned a primary

practice location at the beginning of the season.  

Practices for Junior Girls' teams 14's, 13's, 12's, 10 and under will start on November 18, 2008.
Practices for Junior Girls' teams 18's and 16's will start on December 2, 2008. 

2008/2009 Junior Girls' Club Volleyball Tryouts:

Age Category 18 and Under, Tryouts on Saturday, November 22, 2008
Tryout location will be at Chandler-Gilbert Community College Williams Campus Gym
7450 East Texas Avenue, Mesa, AZ  85212
11:30AM Registration - Sign-in with a $15 Try-Out Fee
12:15PM Parent Meeting
12:00PM - 3:00PM Tryouts
A parent  must be present during the parent meeting and during the end of the try-out session.
Registration and a non-refundable commitment fee of $325 will be collected after selection
of teams towards the end of the tryout process.
 
 
Age Category 16 and Under, Tryouts on Saturday, November 22, 2008

All players must download and bring all the following forms filled out to Tryouts:

1. Molten Volleyball Club Player Info and Tryout Waiver
2. AZ Region Membership Form (2009)
3. AZ Region Medical Release Form (2009)
4. Molten Volleyball Club Photo and Internet Release Form
5. Player Participation Agreement/Commitment Form
6. Player Chandler-Gilbert Release Form
7. Copy of birth certificate (unless you are a returning Molten Volleyball Club player copy is already on file)
8. Bring at least one parent for parent meeting
9. Junior Girls 2008-2009 Fees for your files
